The Neurosurgery Team at KLE’S Dr Prabhakar Kore hospital, Nehru Nagar, Belagavi surgically excised a large Left sided Pontine cavernous hemangioma in a 40 year old female hailing from Gokak who had presented to the hospital with complaints of severe head ache and vomiting with imbalance while walking and facial and limb motor and sensory disturbances. MRI Brain showed a 3cm*3cm large lesion in left Pons extending to middle cerebellar peduncle with mixed ages of bleed.
She successfully underwent the Cranial surgery – microscopic complete excision of Brainstem Cavernous Hemangioma. Post operatively, she was significantly relieved from her previous symptoms and she was discharged on 5th post-day in stable condition, returning to her routine activities at home.
Brainstem Cavernomas are rare in nature and known to have worsening life threatening symptoms due to rebleeds since the lesion is made of ill formed, pathological vascular structure and causes significant neurological deficits and also death if not treated at right time. It carries a lethal prognosis if the severity of the bleed is tremendous during the early episodes or if its located in a very sensitive location like in Brainstem like in her condition. It needs intense preoperative planning, sound anatomical knowledge and precise surgical skills for a favorable outcome.
